Information for Physicians
We offer a FastTrack referral program to physicians who need to refer patients for a quick visit to complete a specific procedure, or to schedule a lengthier visit for a more comprehensive evaluation. Physicians who take advantage of the FastTrack referral program can request that patients be scheduled for their specific procedures within three days.
To request a FastTrack procedure, complete the FastTrack referral form and call the Pain Clinic at (801) 268-7725. A staff member from the Pain Clinic will contact the patient to schedule the requested procedure.
