Cardiac Rehab
The purpose of Cardiac Rehabilitation is to provide the recovering heart patient with the environment necessary to attain personal recovery and health maintenance goals.
Location:
North Medical Office Building Suite 3000.
Hours of Operation:
8:30am - 5:00pm. Tues & Thurs are reserved for new Cardiac Rehab patients. Mon, Wed, Fri are exercise/rehabilitation days for established Cardiac Rehab patients.

Requirements for Registration:
Insurance card copy, Valid photo ID, list of medications prescribed to patient including dosage, and a referral from Primary Care Physician or Cardiologist.
Deposits for Treatment:
Copayments are due at time of service. These vary based on type of insurance, and which phase of rehabilitation a patient is in.
Special Requirements:
Cardiac Rehab treats patients based on an existing cardiac problem. If a recent cardiac procedure has been performed, our Cardiac Rehab unit needs to be informed of the Hospital, date of service, specific procedure, and attending Physician/Cardiologist involved in this procedure.
