Requirements for Registration:
Insurance card copy, Valid picture ID, Physician orders including diagnosis & procedure. Authorization information may be required by your insurance.
Deposits for Treatment:
Dependent upon type of insurance. $25 deposit for self-pay patients. Patients referred to cashier post-treatment.
Special Requirements:
Some procedures require a preparation period such as fasting. Please follow physician's instructions, or call Radiology for assistance. (268-7100)
