My Medical Records

St. Mark’s Hospital values your privacy rights and takes many precautions to safeguard your health information. We want the right people to get information necessary for your care, and make your access to information as convenient as possible.

Your medical record contains documentation by all physicians and healthcare providers involved in your care and treatment while you were here at St. Mark’s Hospital. It contains the results of diagnostic tests and therapeutic procedures performed, as well as information about your medical history, and your condition and progress during your hospitalization.

How Do I Receive My Medical Record Information?

Your medical record, and the information it contains, is confidential. Therefore, we must confirm your identity before releasing your medical record. You can obtain the necessary authorization form at the Information Desk in the front lobby of St. Mark’s Hospital or you can download the authorization form (PDF - 98 KB). Federal HIPAA laws require an authorization to be completed for all medical record requests unless your provider is requesting them for your care. Only the patient or his/her legal guardian can sign the form.

To obtain a copy of your medical record, drop off the completed and signed forms at the Information Desk in the main lobby of St. Mark’s Hospital Monday – Friday between 8:30 – 5:00. Or you may also fax the completed/signed form to 877-865-9738.

Please be aware that it may take 5–7 business days to receive your records. There will be a charge of $0.25 per page. Once the record has been processed, you will receive an invoice for the total amount due.

Because our goal is to get your health information to the appropriate health care provider as quickly as possible, we recommend that your physician (or other healthcare provider) request a copy of your medical records to be delivered directly to his/her office. There is no charge to you for this service.

Behavioral Health/Psychiatric records require additional processing and must be approved by your physician. This process may take up to 45 days.
