The Transitional Care Center (TCC) at St. Mark’s Hospital operates as a transition from acute care to a more independent setting. TCC will help you when your physician determines that you no longer require acute hospitalization, but you still need skilled nursing or rehabilitative care before you can safely return home.
A leader in quality care, the TCC is a licensed 14-bed skilled nursing facility specializing in providing sub–acute level skilled nursing and rehabilitation services. Our convenient hospital–based location affords you the care and resources not available in other settings.
While most patients’ average length–of–stay is less than 14 days, the actual time in our unit may vary. This is primarily determined by your diagnosis, overall condition and progress. Patients benefit from individualized care plans developed by an interdisciplinary team of medical and therapy experts.

Your physician may choose to coordinate your care while on the TCC or our physicians specializing in internal medicine and care of senior adults are also available to provide care.
